  Epic Travel Staffing is hiring a--Travel - Physical Therapist - Home HealthShift: Days / 8a-5:30p, 8x5, 40hrs per week, M-FLength: 7 weeksWeekend/Holiday: PT's rotate one Saturday every 6-8 weeks. (the Sat. they work would be given a day off during the week) Saturday only. No holidayRequirements:
  • WI license
  • 1 year experience
  • Minimum Experience: Home Health experience, training working with the adult/geriatric population with a variety of orthopedic, neurological and functional mobility diagnosis
  • Knowledge of Physical Therapy standards and practices acquired through a minimum of a Masters degree in Physical Therapy
  • Master's Degree in Physical Therapy required
  • AHA BLSOther details:
    • Charting/Equipment: Laptop for point of service documentation, NetSmart EMR, and cell phone for GPS and communication
    • Unit Specific Skillset/Unit Overview: Evaluates, treats and plans patient care in accordance with Physical Therapy Standards of practice.-- Population served includes adult and geriatric patients.-- Documents to meet regulatory and reimbursement requirements.-- Meets productivity expectations of an average of 4.5 visits per day
